Opportunity Description

The Physician Assistant role will be responsible for providing primary medical care and care management/coordinator to a panel of assigned members residing in the contracted Skilled Nursing Facility.

Job Duties

  •  Position is responsible for providing primary medical care and care management/coordination to a panel of members residing in the contracted Skilled Nursing Facility.
  • S/he will collaborate with the member’s PCP and the nursing home Medical Director. S/he will conduct regular and acute visits / assessments in accordance with the Medical Staff by-laws of the Skilled Nurse Facility (SNF) and current Federal and State Health Code.
  • S/he will work with the nursing home ICT (Interdisciplinary Care Team) members.

 

 

Education

Graduate of approved Physician Assistant program. 

 

 

 

Experience & Skills Required

Minimum of two years experience as a Physician Assistant, preferably in clinical geriatric care.

Ability to work independently, develop a differential diagnosis and initiate treatment.

 

 

Certifications

Current NYS PA license/registration, current certification by the NCCPA, current DEA Certificate, BCLS.
