Opportunity Description

The role of the Nurse Manager at a Certified Home Health Agency a world-renowned long-term care organization located in North Bronx, under the direction of the Director of Nursing will be responsible for the coordination of the healthcare and wellness program as well as the personal care services to residents of the organizations EHP/ALR/EALR/SNALR/ALP model. This position is responsible for clinical assessments, oversight, and implementation of the approved individualized service plans as developed by the resident/family and physician to maintain the resident safely within the facility. This position will be Monday through Friday 9:00am-5:00pm Must be Flexible.

Job Duties

The AL-Nurse Manager will ensure that the plan is appropriate and responsive to the resident’s health care needs; as well as reevaluate clinical assessments and make any necessary changes to the plan. Provides oversight and supervision to the program nurses and home health aides provided by contracted agencies.

 

Duties & Responsibilities:

 

  • Responsible for the healthcare implementation of health-related services of the ALR and ALP
  • Participates in assessment and screening of applicants for appropriateness in accordance with policy, standards, rules and regulations;
  • Participates in policy and procedure development, making recommendations to the Director of Nursing as necessary;
  • Provides assessments and reassessments for residents, assesses health needs and identifies the appropriate level of care required by the individual.
  • Document as needed on resident care or changes in status. 
  • Assist in the development and implementation of the individualized service plan for the resident based on needs and findings of the assessment.

Education

Graduate of an accredited school of Nursing with a valid and current New York State License and is currently registered as a licensed registered nurse pursuant to Article 139 of the New York State Education law.

Experience & Skills Required

Minimum of two years of experience and/or training in providing services to older adults and/or residents;  Related work experience may include but need not be limited to home care, residential care, adult care facilities, rehabilitation, health care management and the provision of nursing care in health care. At least 2 years of supervisory experience.
