Nurse Practitioner (NP) – Full-Time
Locations: Brooklyn & Bronx (Urgent Need in Bronx)
Schedule: Monday – Friday, 9 AM – 5 PM (One Late Night per Week, 10 AM – 6 PM)

About the Role
HumanEdge is actively seeking two experienced Nurse Practitioners (NPs) to join a dedicated healthcare team in Brooklyn and the Bronx. These full-time positions provide an excellent opportunity for compassionate, patient-focused NPs to work in a Federally Qualified Health Center (FQHC) setting, providing high-quality medical care to individuals with developmental disabilities.
Our ideal candidates are goal-oriented, solution-driven professionals who can work independently while collaborating with a multidisciplinary team to develop and implement personalized medical care plans.
Key Responsibilities
- Conduct comprehensive physical exams annually and as needed, including vision and hearing screenings
- Prescribe medications, devices, and immunizations in accordance with clinical protocols
- Communicate with patients, families, and caregivers regarding medical emergencies or significant changes in health status
- Collaborate with rehabilitation staff to develop and monitor individualized habilitation programs
- Document patient progress, history, and physical exams in Electronic Medical Records (EMR)
- Coordinate with hospitals, clinics, and specialists to ensure continuity of care
- Order and evaluate diagnostic tests, labs, and screenings
- Develop treatment plans based on patient needs and standard NP protocols
- Participate in quality improvement initiatives to enhance patient care and outcomes

Education
Current Nurse Practitioner (NP) license in New York State
Experience & Skills Required
- Minimum of one year of experience working with individuals with developmental disabilities
- Proficiency with EMR and e-prescribing systems
- Strong clinical judgment, communication, and teamwork skills
- Ability to adapt to a fast-paced healthcare environment
Certifications
- Certification through ANCC or AANP
- CPR Certification, Infection Control Certificate, and DEA Certificate required
